THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Uses & Disclosures

Your treating provider and NuMi Rx (as a Business Associate) may use and disclose your PHI for treatment, payment, and healthcare operations, and as otherwise permitted or required by law (e.g., public health reporting, court orders).

Your Rights

Right to inspect and copy your records.
Right to request amendments.
Right to an accounting of disclosures.
Right to request restrictions and confidential communications.
Right to a paper copy of this Notice.
Right to be notified of a breach of unsecured PHI.

Authorization Required

Most uses of psychotherapy notes, marketing communications, and sales of PHI require your written authorization, which you may revoke at any time.

Complaints

You may file a complaint with our Privacy Officer at privacy@numirx.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
